Has been in effect since September 2013, it became a requirement that any provider needs to have this on file with all that are either working in the office or sales who come into your office.  This is the responsibility of the provider, not the sales team or your team members.  After going into hundreds of offices across our country I have found that only  2 out of over 40 of our providers have one or know they need one.
